Key Details

Preparation & Timing
- No special preparation is needed.
- Do not collect if you are sick.
Collection Instructions
- Rinse your mouth with cold water.
- Open one of the sleeves and remove one swab.
- Keep the package intact.
- Only peel it open enough to remove the applicator.
- Remove ONE cotton swab applicator.
- Do NOT touch the cotton tip.
- Swallow to remove excess saliva.
- Using a circular motion, rub the swab on the inside of one cheek about 20 times.
- Use enough pressure for the cheek to be pushed outward.
- Gently wave the swab in the air to let it dry for 3 minutes.
- Do NOT touch the swab.
- Once the swab is dry, place it back in the sleeve.
- Remove the second swab from the sleeve.
- Repeat steps 4-8 for the second swab.
- Use the same cheek.
- Once you have completed the first set of swabs, open the other sleeve.
- Repeat steps 2-9 for the second two swabs.
- Use the other cheek.
Example 1st swab Right cheek 2nd swab Right cheek 3rd swab Left cheek 4th swab Left cheek - Once all four samples have been collected, place the two sleeves containing the four dry swabs in the provided Swab Collection Return Envelope.
- Do NOT place the swabs in a plastic bag.
Requisition & Payment
- Your kit has been prepaid. You DO NOT need to pay anything extra.
- Instructions on how to fill out your requisition can be found HERE.
- Since you pre-paid for your tests, you do not need to fill in the payment information on any of these forms.
- DO NOT fill out any information on gplforms.com.
Shipping
- Ship samples as soon as possible after collection.
- To prepare your samples for shipping,
- Place the two swab-containing sleeves into the provided Swab Collection Return Envelope.
- Seal the envelope.
- Place the Swab Collection Return Envelope into the provided FedEx envelope.
- Place the completed test requisition form into the FedEx envelope.
- Place the Informed Consent form into the FedEx envelope.
- Seal the envelope.
- Make a note of the tracking number so that you can track your sample back to the lab.
- Ship your samples via FedEx.
- You can take your samples to the nearest FedEx location.
- DO NOT place your samples in a FedEx drop box.
- You can also schedule a FedEx pickup by calling 1-800-463-3339.
- At the automated greeting, say, "schedule a pickup."
- At the next prompt, say, "schedule a pickup with a label or stamp."
- You will be asked if the waybill says "stamp" - say, "yes."
- Then give your address information.
Results
- Results should be released to your provider 5-6 weeks after samples are received at the lab. We cannot guarantee processing time.
- Your provider will notify you when your results are in and schedule a follow-up appointment for you to review them together.
- Your provider may choose to release your results to you. Rupa cannot release your results without provider authorization.
