Key Details
- Blood sample collection must be performed by a phlebotomist, Monday - Thursday only.
- Information about scheduling a blood draw can be found HERE.
- You may need to pay a separate fee for phlebotomy.
- Fast for 12 hours prior to your blood draw.
- Ship sample Monday - Thursday only.
Preparation & Timing
- Blood sample collection must be performed by a phlebotomist, Monday - Thursday only.
- Collect your urine sample prior to your blood draw.
- Fast for 12 hours prior to your blood draw.
- Freeze the provided ice pack for at least 4-6 hours prior to your blood draw.
Collection Instructions
Urine Collection Instructions
- Collect urine in the provided collection cup at any time of the day.
- Use the provided pipette to transfer 5 mL of urine to the yellow top transfer tube.
- Write the following information on the tube using a permanent marker.
- Patient's first and last name
- Patient's date of birth
- Date of collection
- Place the yellow top transfer tube in the freezer.
- Bring the frozen urine sample with you to your blood draw.
Blood Collection Instructions
- Bring the following items with you to your blood draw appointment.
- Entire test kit
- Frozen ice pack
- Frozen urine sample
- Completed test requisition form
- The phlebotomist will collect, prepare, and ship your sample for you.

Requisition & Payment
- Your kit has been prepaid. You may need to pay a separate fee for phlebotomy.
- Instructions on how to fill out your requisition can be found HERE.
- Since you pre-paid for your tests, you do not need to fill in the payment information on any of these forms.
Shipping
- The phlebotomist will ship your sample for you using the materials provided in the test kit.
- Samples should be shipped Monday - Thursday only.
Results
- Results should be released to your provider 7-14 business days after samples are received at the lab. We cannot guarantee processing time.
- Your provider will notify you when your results are in and schedule a follow-up appointment for you to review them together.
