Key Details 

  • Sample collection must be performed by a phlebotomist, Monday - Friday only. 
    • You WILL NOT need to pay a separate fee for phlebotomy.
  • You WILL NOT receive a test kit for your blood draw.
  • Your requisition form will be emailed to you. You must bring your requisition form (printed or on your phone) with you to your blood draw. 
  • Your blood draw will be performed at Labcorp. Rupa Health recommends making an appointment for your blood draw.
  • You must fast for 12-14 hours prior to sample collection.

Collection Instructions

  1. Make an appointment at Labcorp.

  2. Bring the following items with you to your blood draw appointment. 
    1. Test requisition form - this form can be printed or shown on your mobile device  
  3. The phlebotomist will collect, prepare, and ship your sample for you.

Requisition & Payment

  • Your kit has been prepaid. You WILL NOT need to pay a separate fee for phlebotomy.
  • Instructions on how to fill out your requisition can be found HERE.

Shipping

  • The phlebotomist will ship your sample for you using the materials provided in the test kit.

Results

  • Results should be released to your provider 1-10 business days after samples are received at the lab. We cannot guarantee processing time. 
  • Your provider will notify you when your results are in and schedule a follow-up appointment for you to review them together.
