The Adrenocortex Stress Profile (ASP) with Cortisol Awakening Response (CAR) measures hormones involved in a patient's natural circadian rhythm to evaluate stress and resiliency of the hypothalamic-pituitary-adrenal (HPA) axis. Key Details 

  • Sample collection is strictly timed.
  • If you are taking ketoconazole, clomiphene, phenytoin, steroids, or DHEA supplementation, please consult your healthcare provider prior to collecting your samples. 
  • For 1 hour prior to each collection, do not eat or drink anything and remove all lip balm and/or lipstick. 
  • Samples should be frozen for a minimum of 2 hours prior to shipping.
  • Ship samples via FedEx as soon as possible, Monday - Thursday only.

Preparation & Timing

  • Sample collection must follow the collection schedule exactly.
    • Consider waking at 6 am on the day of collection.
  • If you are taking the following medications, please consult your healthcare provider prior to the test: ketoconazole, clomiphene, phenytoin, steroids, and DHEA supplementation.
    • Do not change or discontinue medications unless instructed by your healthcare provider. 
  • Do not eat or drink anything except water for 1 hour prior to EACH collection.
  • Remove all lip balm and lipstick 1 hour prior to EACH collection.
  • If you have difficulty producing enough saliva during the test, try these methods to quickly produce saliva:
    • Rinse your mouth with water and spit out completely.
    • Press the tip of your tongue to the roof of your mouth against your teeth.
    • Think of sour foods.
    • Yawn.

Collection Instructions

  1. Write your first and last name, date of birth, gender, and dates of collection on the test requisition form. 
  2. Collect samples according to the following schedule:
    1. Immediately upon waking: fill one blue tube
    2. 30 minutes from the end of the first collection: fill the second blue tube
    3. 7:00 am - 9:00 am: fill one red tube
    4. 11:00 am - 1:00 pm: fill another red tube
    5. 3:00 pm - 5:00 pm: fill the third red tube
    6. 10:00 pm - 12:00 am: fill the final red tube
  3. Fill the correct tube (see above) with saliva to the designated level, without bubbles or mucus, within 5 minutes.
  4. Replace the cap tightly.
  5. Write your first and last name, date of birth, and the start and stop collection times on the label. Attach the label to the collection tube. 
  6. Freeze the tube immediately
    1. Samples should be frozen a minimum of 2 hours prior to shipping.
    2. Keep samples frozen until ready to ship.
  7. Repeat steps 3-6 for each collection.
  8. When you are ready to ship the samples, place the frozen samples into the biohazard bag.
  9. Place the biohazard bag into the clamshell container.
  10. Place the clamshell container and completed test requisition form into the provided, prepaid mailing envelope.

Requisition & Payment

  • Your kit has been prepaid. You DO NOT need to pay anything extra.
  • Instructions on how to fill out your requisition can be found here for insurance pay and here for cash pay.
    • Since you pre-paid for your tests, you do not need to fill in the payment information on any of these forms.

Shipping

  • Samples should be frozen for at least 2 hours prior to shipping.
  • Use the shipping materials provided. 
    • Place the samples in the biohazard bag.
    • Place the biohazard bag into the clamshell container. 
    • Place the container and completed requisition form into the provided, prepaid mailing envelope. 
  • Ship via FedEx as soon as possible.
  • Ship samples Monday - Thursday only.

Results

  • Results should be released to your provider 7-21 days after samples are received at the lab. We cannot guarantee processing time. 
  • Your provider will notify you when your results are in and schedule a follow-up appointment for you to review them together. 
  • Your provider may choose to release your results to you. Rupa cannot release your results without provider authorization.
