Collection Instructions

  1. Write the following information on the blood spot card.
    1. Patient's full name
    2. Date of collection
  2. Wash your hands using warm water. 
  3. Dry your hands with a clean towel. 
  4. Move your arms around and rub your hands together (to improve circulation).
  5. Determine which finger you will stick. 
  6. Clean the finger with the alcohol prep pad. 
  7. Open the lancet by twisting off the tab.
  8. Hold the lancet firmly against the puncture site.
  9. Press the lancet firmly into the finger until you hear a click. 
  10. Position a blood drop above one of the circular pads on the card. 
  11. Allow the blood to drip onto and absorb into the card.
    1. DO NOT touch the card with your finger. 
    2. Circles should be filled completely.
    3. Blood should be soaked through to the other side of the card. 
  12. How many circles you will fill depends on your test. See the chart below. 
  13. When you have filled the required number of circles, apply the bandage to the puncture site.
  14. Bend the blood spot card away from its cover. 
  15. Allow the card to AIR DRY completely. 
    1. DO NOT use heat to dry the card. 
  16. Once the blood spot card is completely dry, place the card into the biohazard bag and seal it. 
  17. Place the biohazard bag into the return envelope. 
  18. Place the completed test requisition form into the return envelope.

Requisition & Payment

  • Your kit has been prepaid. You DO NOT need to pay anything extra.
  • Instructions on how to fill out your requisition can be found HERE.
    • Since you pre-paid for your tests, you do not need to fill in the payment information on any of these forms.

Shipping

  • Ship samples back to the lab as soon as possible.
  • To prepare your sample for shipping,
    • Make sure the blood spot card is completely dry. 
    • Place the blood spot card into the biohazard bag.
    • Seal the biohazard bag. 
    • Place the biohazard bag into the return envelope. 
    • Place the completed test requisition form into the return envelope. 
    • Seal the return envelope and ship using the indicated carrier. 
  • Make a note of the tracking number on the return mailer so that you can track your sample back to the lab.

Results

  • Results should be released to your provider 5-7 days after samples are received at the lab. We cannot guarantee processing time. 
  • Your provider will notify you when your results are in and schedule a follow-up appointment for you to review them together.
