Key Details 

  • Do not collect samples while menstruating.
  • Avoid apples, grapes (including raisins), pears, and cranberries and any juices or products made with them for 48 hours prior to collection.
  • Avoid arabinogalactan, echinacea, reishi mushrooms, and ribose supplements for 48 hours prior to collection. 
  • Limit liquid intake starting at 6pm the night prior to collection. 
  • You will collect your sample upon waking. Do not eat or drink prior to collection.
  • Freeze your sample before shipping.
  • Your sample must be shipped in the cardboard box and with the gel freezer pack.
  • Samples may only be shipped Monday through Thursday. 
  • Do NOT fill out any information on the gplforms.com website.

Preparation & Timing

Collection Instructions

  1. Save the cardboard box.
  2. Place the gel pack flat into the freezer for at least 4 hours.
  3. Collect the first morning urine in the provided collection cup
  4. Screw collection cup lid on tightly.
  5. Freeze urine specimen for a minimum of 4 hours before shipping. 
    1. Keep in the freezer until ready to ship. 
  6. Place the collection cup into the biohazard zip-lock bag along with the absorbent packing sheet.
  7. Place the frozen gel pack and the sample into the silver thermo bag.
  8. Place the silver thermo bag into the box.
    1. If the sample is not in the box, it will be rejected by FedEx.
  9. Place the completed test requisition form into the box and close the lid.
  10. Place the box into the FedEx Clinical Pak Mailer and seal it tightly.

Requisition & Payment

  • Your kit has been prepaid. You DO NOT need to pay anything extra.
  • Instructions on how to fill out your requisition can be found HERE.
    • Since you pre-paid for your tests, you do not need to fill in the payment information on any of these forms.
    • Do NOT fill out any information on the gplforms.com website.

Shipping

  • Ship the sample using the provided packing materials.
    • Place the collection cup into the biohazard zip-lock bag along with the absorbent packing sheet.
    • Place the frozen gel pack and the sample into the silver thermo bag.
    • Place the silver thermo bag into the box. If the sample is not in the box, it will be rejected by FedEx.
    • Place the completed test requisition form into the box and close the lid.
    • Place the box into the FedEx Clinical Pak Mailer and seal it tightly.
  • Ship the sample using the provided, prepaid FedEx Clinical Pak Mailer.
    • To schedule a FedEx pickup, call 1-800-463-3339
      • When the automated greeting begins, say, “Schedule a pickup.”
      • When asked if your label has the word “Stamp” on it, say, “Yes.”
      • Let them know you are shipping using a Billable Stamp.
      • Your tracking number can be found on the Return Label.
  • Samples may only be shipped Monday through Thursday
  • Do not put the sample in a FedEx drop box.
  • Make a note of your tracking number (found on the Return Label) if you would like to track your sample’s shipping progress.

Results

  • Results should be released to your provider 7-21 business days after samples are received at the lab. We cannot guarantee processing time. 
  • Your provider will notify you when your results are in and schedule a follow-up appointment for you to review them together. 
  • Your provider may choose to release your results to you. Rupa cannot release your results without provider authorization.
