Key Details 

  • Do not collect samples while menstruating.
  • Limit liquid intake starting at 6 pm the night prior to collection. 
  • You will collect your sample upon waking. Do not eat or drink prior to collection.
  • Samples may only be shipped Monday through Thursday. 
  • Do NOT fill out any information on the gplforms.com website.

Collection Instructions

  1. Place the gel pack into the freezer for at least 4 hours prior to shipping.
  2. Collect your first morning urine in the provided collection cup
  3. Screw the collection cup lid on tightly.
  4. Freeze urine specimen for at least 4 hours prior to shipping. 
    1. Keep in the freezer until ready to ship. 
  5. Place the collection cup into the biohazard zip-lock bag along with the absorbent packing sheet.
  6. Place the frozen gel pack and the biohazard bag into the silver thermo bag.
  7. Place the silver thermo bag into the cardboard box.
    1. If the sample is not in the box, it will be rejected by FedEx.
  8. Place the completed test requisition form into the cardboard box and close the lid.

Requisition & Payment

  • Your kit has been prepaid. You DO NOT need to pay anything extra.
  • Instructions on how to fill out your requisition can be found HERE.
    • Since you pre-paid for your tests, you do not need to fill in the payment information on any of these forms.
    • Do NOT fill out any information on the gplforms.com website.

Shipping

  • Samples may only be shipped Monday through Thursday. 
  • To prepare your sample for shipping.
    • Place the collection cup into the biohazard zip-lock bag along with the absorbent packing sheet.
    • Place the frozen gel pack and the biohazard bag into the silver thermo bag.
    • Place the silver thermo bag into the cardboard box. 
      • If the sample is not in the box, it will be rejected by FedEx.
    • Place the completed test requisition form into the cardboard box and close the lid.
    • Place the cardboard box into the FedEx Clinical Pak Mailer and seal it tightly.
  • Ship the sample using the provided, prepaid FedEx Clinical Pak Mailer.
    • To schedule a FedEx pickup, call 1-800-463-3339
      • When the automated greeting begins, say, “Schedule a pickup.”
      • When asked if your label has the word “Stamp” on it, say, “Yes.”
      • Let them know you are shipping using a Billable Stamp.
      • Your tracking number can be found on the Return Label.
    • You can also take your sample to the nearest FedEx location.
    • Do not put your sample in a FedEx drop box.
  • Make a note of your tracking number (found on the Return Label) so that you can track your sample back to the lab.

Results

  • Results should be released to your provider 7-21 business days after samples are received at the lab. We cannot guarantee processing time. 
  • Your provider will notify you when your results are in and schedule a follow-up appointment for you to review them together. 
  • Your provider may choose to release your results to you. Rupa cannot release your results without provider authorization.
