Collection Instructions

  1. Collect urine in the provided urine collection cup.
  2. Open the cap on the specimen tube. 
  3. Pour or pipette the urine from the cup into the tube.
  4. Replace the cap and seal tightly. 
  5. Write the patient's name and date of birth on the tube label.
  6. Place one frozen ice pack on the bottom of the styrofoam cooler. 
  7. Place the sample tube in the biohazard bag and seal. 
  8. Place the completed test requisition form into the back pocket of the biohazard bag. 
  9. Place the biohazard bag in the styrofoam cooler.
  10. Place one frozen ice pack on top of the biohazard bag in the styrofoam cooler.

Requisition & Payment

  • Your kit has been prepaid. You do NOT need to pay anything extra.
  • Instructions on how to fill out your requisition can be found HERE.
    • Since you pre-paid for your tests, you do not need to fill in the payment information on any of these forms.

Shipping

  • Ship samples as soon as possible.
  • To prepare your sample for shipping,
    • Place one frozen ice pack on the bottom of the styrofoam cooler. 
    • Place the sample tubes in the biohazard bag and seal. 
    • Place the completed test requisition form into the back pocket of the biohazard bag. 
    • Place the biohazard bag in the styrofoam cooler.
    • Place one frozen ice pack on top of the biohazard bag in the styrofoam cooler. 
    • Place the styrofoam cooler in the shipping box. 
  • Ship with FedEx using the provided, prepaid stamp.
    • Make a note of the tracking number so that you can track the sample back to the lab.

Results

  • Results should be released to your provider 7-14 days after samples are received at the lab. We cannot guarantee processing time. 
  • Your provider will notify you when your results are in and schedule a follow-up appointment for you to review them together.
