Key Details
You must collect your swab samples prior to your blood draw.
Bring your swab samples to your blood draw appointment.
You must avoid certain activities the night prior to and the day of collection.
Samples should be collected upon waking.
Preparation & Timing

The night before you collect your samples, do not use mouthwash.
Avoid the following activities prior to sample collection.
Eating
Drinking anything but water
Brushing your teeth
Samples should be collected upon waking.
Collection Instructions
Collect your samples immediately upon waking (before eating, drinking, brushing your teeth, etc.).
Thoroughly wash your hands with soap and warm water.
Dry your hands with a clean towel.
Gently peel open the package labeled "Sterile Cotton Tipped Applicator."
Keep the package intact.
Only peel it open enough to remove the applicator.
Remove ONE cotton swab applicator.
DO NOT touch the cotton tip.
Open your mouth widely.
Insert the cotton tip of the swab applicator.
Aggressively scrape the inside of your cheek for at least 30 seconds.
Use a back and forth, up and down motion.
Rotate the applicator several times.
Swab between the cheek and gums.
Avoid excessive saliva.
Allow the swab to air dry for 15-20 minutes.
DO NOT let the cotton tip touch any surface or your hands.
Repeat steps 5-9 with the second swab.
Complete the test registration (instructions HERE).
Please ensure that the name on the registration matches the name on your Rupa order EXACTLY. If it does not match, you may need to recollect your samples.
Once the swabs have dried, place them back into the applicator package, cotton tip first.
Write the following information on the label and place it on the envelope.
Patient's full name
Patient's date of birth
Insert the swab applicator package into the provided envelope and seal.
Place the envelope containing the swabs in the collection box and bring it to your blood draw.
Requisition & Payment
Your kit has been prepaid. You may need to pay an additional fee for phlebotomy (for the main NutrEval® test).

Since you pre-paid for your tests, you do not need to fill in the payment information on this form.
Shipping
Make a note of the tracking number on the return label so you can track your sample back to the lab.
Bring your test kit, frozen urine samples, and swabs to your blood draw appointment.
Place the following items into the biohazard bag.
Frozen urine tubes
Frozen freezer brick
Absorbent material
Place the biohazard bag into the insulator box.
Place the insulator box into the outer collection box.
Place the swabs into the provided envelope and seal.
Place the envelope containing the swabs into the outer collection box.
The phlebotomist will ship your samples for you with the materials included in the test kit.
Rupa Health recommends getting your blood drawn and samples shipped Monday - Thursday only to avoid shipment delays.
Results
Results should be released to your provider 7-21 business days after samples are received at the lab. We cannot guarantee processing time.
Your provider will notify you when your results are in and schedule a follow-up appointment for you to review them together.
