    3. Date of collection

  2. Tear open the alcohol swab, gauze, and bandage packages. Leave the items inside.

  3. Unfold the collection card to expose the blood collection card. DO NOT touch the collection circles.

    1. Tip: You can stabilize the card by taping the outer cover of the card to the edge of a tabletop.

  4. Wash your hands under warm water for 30-60 seconds.

  5. Twist off the blue tab to break the seal of the lancet.

    1. DO NOT touch the white tip.

  6. Clean the tip of the middle or ring finger on your non-dominant hand with the provided alcohol pad.

  7. Try one of the following activities to increase circulation.

    1. Rubbing your hands together

    2. Swinging your arms around several times

    3. Shaking your arms

  8. Place the tip of the lancet against the side of your fingertip.

  9. Press down firmly until you hear a click.

  10. Gently press the pricked finger from below the puncture site to allow a large drop of blood to form.

    1. AVOID strong repetitive pressure on the pricked finger. This may cause contamination of the sample

  11. Wipe away the first drop of blood with the gauze.

  12. Hold the finger downward to allow the blood to drop onto the collection card circles.

    1. DO NOT touch the card with your finger.

    2. DO NOT rub or smear the sample.

    3. Make sure the blood saturates through to the back of the card.

  13. Repeat step 12 until you have filled in the required number of circles (this number can be found on the test requisition form).

    1. Filling more than the required number of spots is recommended (if possible) to ensure enough blood is present.

    2. If you have more than one test or a test measuring many analytes, you may need to fill spots on two cards. Consult your requisition form to determine if this applies to you.

  14. Once the collection is completed, apply the bandage to the puncture site.

  15. Complete the test requisition form.

    1. Please ensure that the information on the blood spot card(s) matches the information on your requisition form EXACTLY. If it does not match, you may need to recollect your sample.

  16. Air-dry the sample for at least 3 hours.

    1. The card can be propped up or left taped to the table.

    2. Blood should NOT be in contact with the cover.

    3. Keep the card away from moisture and sunlight.

  17. Once the card is dried, tuck the cover into the flap as indicated on the card.
