Key Details
🧪 You will collect two samples: a urine sample and a blood sample.
🗓️ Collect your urine sample before your blood draw.
🏥 Sample collection must be performed by a phlebotomist, Monday - Thursday only.
💰 You may need to pay a separate fee for phlebotomy.
❌ You must avoid certain foods for 48 hours before collecting your urine sample.
🍽️ You must fast for 8 hours before your blood draw.
🧊 Your urine sample should remain in the freezer until you ship it.
📄 Please print the requisition form from your Rupa patient portal and send it back with your samples. Your sample must be CLEARLY labeled and arrive at the lab with a completed test requisition form for the lab to analyze it.
🚚 Ship your sample as soon as possible after collection.
Preparation & Timing
You will collect two samples: a urine sample and a blood sample.
Collect your urine sample before your blood draw.
Sample collection must be performed by a phlebotomist, Monday - Thursday only.

Avoid the following foods for 48 hours before collecting your sample.
Fruits
Jams
Jellies
You must fast for 8 hours before your blood draw.
Continue taking any nutritional supplements and medications unless your healthcare provider says otherwise.
Collection Instructions
Urine Collection
Write the following information on the collection cup label using a permanent marker. Unlabeled samples will not be analyzed by the lab.
Patient's full name
Patient's date of birth
Date of collection
Before bed, empty your bladder. DO NOT collect this urine.
Upon waking, collect your first morning urine into the provided collection cup.
Ensure it has been at least six hours since you last emptied your bladder.
You should collect between 15 mL and 50 mL of urine.
DO NOT collect more than 50 mL of urine.
Place the cap back on the collection cup and close it.
Turn the cap until you hear 2 clicks. It is not fully closed until you hear the 2 clicks.
Wipe any drops of urine off the outside of the cup.
Place the sample into the provided specimen bag.
Seal the bag.
Place the specimen bag into the freezer for at least 2 hours.
The urine sample should stay in the freezer until it is shipped.
Complete the test requisition form.
NOTE: Your sample will not be analyzed by the lab if it arrives at the lab without a completed test requisition form.
Blood Collection
Read this article to prepare for your blood draw.
You must get your blood drawn Monday - Thursday.
Complete the test requisition form.
NOTE: Your sample will not be analyzed by the lab if it arrives at the lab without a completed test requisition form.
Bring the following items with you to your blood draw appointment.
Test kit
Frozen urine sample
Completed test requisition form
The phlebotomist will collect, prepare, and ship your sample for you.
Please make sure the phlebotomist CLEARLY labels your sample. Unlabeled samples will not be analyzed by the lab.
Requisition & Payment
Your kit has been prepaid. You DO NOT need to pay anything extra.
Please print the requisition form from your Rupa patient portal and send it back with your samples.
NOTE: Your sample will not be analyzed by the lab if your requisition form is not completed and sent with your sample.
You must complete your requisition form and send it back with your samples.
Since you pre-paid for your tests, you do not need to fill in the payment information on this form.
Shipping
Make a note of the tracking number on the return mailer so that you can track your sample back to the lab.
The phlebotomist will ship your sample for you using the materials provided in the test kit.
Results
Results should be released to your provider 5-10 business days after your sample is received at the lab. Rupa Health cannot guarantee processing time.
