Quest Diagnostics's blood panels can include any combination of biomarkers chosen for you by your practitioner.

- Sample collection must be performed by a phlebotomist, Monday - Friday only.
- You WILL NOT receive a test kit for your blood draw.
- Your requisition form will be emailed to you. You must bring your requisition form with you to your blood draw.
- Your blood draw will be performed at Quest Diagnostics. Rupa Health recommends making an appointment for your blood draw.
- You may be required to fast for at least 12 hours before sample collection.

- 25-Hydroxy Vitamin D
- Comprehensive Metabolic Panel
- Direct LDL
- HDL
- HDL Cholesterol
- Insulin
- Iron
- Iron and Total Iron Binding Capacity
- Lipid Panel
- Lp-PLA2 Activity
- Omega-3 and -6 Fatty Acids
- Prolactin
- Renal Function Panel
- Total Cholesterol
- Transferrin
- Triglycerides
- Vitamin A
- Vitamin B6
- VLDL Cholesterol

YOUR REQUISITION FORM CAN BE FOUND IN YOUR RUPA PATIENT PORTAL. RUPA HEALTH WILL ALSO EMAIL IT TO YOU.
