Key Details
- You will collect two samples: a urine sample and a blood sample.
- Collect your urine sample prior to your blood draw.
- Blood sample collection must be performed by a phlebotomist, Monday - Thursday only.
- Information about scheduling a blood draw can be found HERE.
- You may need to pay a separate fee for phlebotomy.
- You must avoid certain foods for 48 hours prior to collecting your samples.
- You must fast for 8 hours prior to your blood draw.
- NOTE: Your samples must be CLEARLY labeled and arrive at the lab with a completed test requisition form for the lab to analyze them.
Preparation & Timing
- Blood sample collection must be performed by a phlebotomist.
- Information about scheduling a blood draw can be found HERE.
- Avoid the following foods for 48 hours prior to collecting your samples.
- Fruits
- Jams
- Jellies
- Fast for 8 hours prior to your blood draw.
- Continue taking any nutritional supplements and medications unless advised differently by your healthcare provider.
Collection Instructions
Urine Collection
- Write the following information on the collection cup label using a permanent marker. Unlabeled samples will not be analyzed by the lab.
- Patient's full name
- Patient's date of birth
- Date of collection
- Complete the test requisition form (patient information, date of collection).

- Place the completed test requisition form into the outside pocket of the specimen bag.
- Prior to bed, empty your bladder. DO NOT collect this urine.
- Upon waking, collect your first morning urine into the provided collection cup.
- Ensure it has been at least six hours since you last emptied your bladder.
- You should collect between 15 mL and 50 mL of urine.
- DO NOT collect more than 50 mL of urine.
- Place the cap back on the collection cup and close it.
- Turn the cap until you hear 2 clicks. It is not fully closed until you hear the 2 clicks.
- Wipe any drops of urine off the outside of the cup.
- Place the sample into the provided specimen bag.
- Seal the bag.
- Place the specimen bag into the freezer. Keep it in the freezer until you go to your blood draw appointment.
- The urine sample should stay cool until it is shipped.
- Ensure the test requisition form is completed with your personal and sample collection information.

- The phlebotomist will fill in the plasma collection information.
- NOTE: Your samples will not be analyzed by the lab if they arrives at the lab without a completed test requisition form.
Blood Collection
- You must get your blood drawn Monday - Thursday.
- Bring the following items with you to your blood draw appointment.
- Test kit
- Completed test requisition form
- Urine sample
- The phlebotomist will collect, prepare, and ship your sample for you.

- Please make sure the phlebotomist CLEARLY labels your sample. Unlabeled samples will not be analyzed by the lab.
Requisition & Payment
- Your kit has been prepaid. You may need to pay a separate fee for phlebotomy.
- Instructions on how to fill out your requisition can be found HERE.
- Since you pre-paid for your tests, you do not need to fill in the payment information on any of these forms.
- NOTE: Your samples will not be analyzed by the lab if your requisition form is not completed and sent with your samples.
Shipping
- Make a note of the tracking number on the return mailer so that you can track your samples back to the lab.
- The phlebotomist will ship your samples for you using the materials provided in the test kit.
Results
- Results should be released to your provider 5-10 business days after samples are received at the lab. We cannot guarantee processing time.
- Your provider will notify you when your results are in and schedule a follow-up appointment for you to review them together.
