Key Details
If you are a menstruating woman, you must collect at a specific time in your cycle.
If you take hormones, you may need to time your cycle appropriately.
You must avoid certain foods and activities for 24 hours prior to sample collection.
Drink a normal amount of water before and during collection.
Keep the cardboard kit box and the insulated cooler that come with your kit.
Keep your samples in the freezer until you are ready to ship.
Ship via FedEx as soon as possible, Monday - Friday only.
Preparation & Timing
If you are a man or post-menopausal woman, you may collect your sample on any day.
If you are a menstruating woman, you must collect at a specific time in your cycle.
Regular cycle: Collect any day between Day 19 and Day 23 of your cycle (for a 28-day cycle).
The first day of your period is Day 1.
Irregular cycle: Contact your healthcare provider for timing guidance.

Topical hormones, injectable hormones, and patch hormones can be used as normal.
DO NOT skip oral birth control pills unless instructed otherwise by your healthcare practitioner.
5 days before sample collection
Stop taking any cortisol-containing products.
Please do not discontinue prescription medications without consulting your healthcare provider.
3 days before sample collection
Stop taking any oral or sublingual hormones.
You may resume taking these hormones once your sample collection is complete.
Please do not discontinue prescription medications without consulting your healthcare provider.
Stop taking any vaginal hormones.
You may resume taking these hormones once your sample collection is complete.

24 hours before sample collection
Drink normal amounts of fluid.
Avoid the following foods and products. Do not consume them again until sample collection is complete.
Alcohol
Caffeine-containing foods and drinks
Tobacco
Nicotine-containing products
Avoid strenuous exercise.
Collection Instructions
NOTE: Collect from a full bladder as much as possible.

Collection Instructions
FOR EACH COLLECTION
Collect a midstream specimen of urine in one of the provided cups.
You may reuse a cup if necessary.
DO NOT urinate directly into the sample tubes.
Pipette the urine into the proper sample tube.
See the schedule above to find the correct tube color for your collection.
DO NOT touch the inside of this tube with your fingers.
Fill the tube up to the "fill line."
Discard any remaining urine in the toilet.
Close the tube securely.
Rock the tube gently.
Write the following information on the sample tube.
Your name
Date of collection
Time of collection
Write the following information on the test requisition form.

Place the urine tube back into the original plastic bag.
Place the plastic bag into the freezer.
(First collection only) Place the provided ice pack in the freezer.
ONCE YOU HAVE COLLECTED ALL OF YOUR SAMPLES
Ensure the test requisition form is complete.
Keep the samples in the freezer with the frozen ice pack for 4-6 hours prior to shipping.
When you are ready to ship, remove the samples from the freezer.
Place the plastic bag containing the frozen urine samples into the provided insulated cooler.
Place the provided frozen ice pack into the provided insulated cooler.
Securely place the lid on the insulated cooler.
Place the insulated cooler into the cardboard kit box.
Place the requisition form into the cardboard kit box.
Close the cardboard box.
Place the cardboard box into the FedEx Clinical Pak.
Requisition & Payment
Your kit has been prepaid. You DO NOT need to pay anything extra.
Instructions on how to fill out your requisition can be found HERE.
Since you pre-paid for your tests, you do not need to fill in the payment information on any of these forms.
Shipping
Ship your sample as soon as possible.
Keep your samples in the freezer until you ship the test kit back to the lab.
Ship samples Monday - Friday only.
To prepare your samples for shipping,
Remove the samples from the freezer.
Place the plastic bag containing the frozen urine samples into the provided insulated cooler.
Place the provided frozen ice pack into the provided insulated cooler.
Securely place the lid on the insulated cooler.
Place the insulated cooler into the cardboard kit box.
Place the completed requisition form into the cardboard kit box.
Close the cardboard box.
Place the cardboard box into the FedEx Clinical Pak.
Place the provided prepaid label onto the FedEx Clinical Pak.
Make a note of the tracking number so that you can track your samples back to the lab.
Ship using FedEx.
DO NOT place your sample in a FedEx drop box.
You can schedule a FedEx pickup by calling 1-800-463-3339.
At the automated greeting, say, "schedule a pickup."
At the next prompt, say, "schedule a pickup with a label or stamp."
You will be asked if the waybill says "stamp" - say, "yes."
Then give your address information.
Results
Results should be released to your provider 3-8 business days after samples are received at the lab. We cannot guarantee processing time.
Your provider will notify you when your results are in and schedule a follow-up appointment for you to review them together.
