DHA Laboratory Blood Tests
Updated over a week ago

DHA Laboratory partners exclusively with Labcorp for their blood draws. Blood tests ordered through DHA Laboratory must be drawn through Labcorp.

How the DHA Laboratory Phlebotomy Process Works

  1. DHA Laboratory will send you an email once your order has been processed. This email will contain preparation instructions, your test requisition form, and the closest Labcorp locations to you.

  2. Print your requisition form.

  3. Schedule an appointment at Labcorp. *Note: You are not required to make an appointment, but Rupa Health recommends it.

  4. Take your requisition form with you to Labcorp for your blood draw.

Will I receive a test kit for my blood draw?

No, you will not receive a test kit for your blood draw. All Labcorp locations already have all of the required collection materials.

How will I receive my requisition form?

DHA Laboratory will send you an email titled "Blood Draw Documents for [Your Initials] (IMPORTANT)" that will include your requisition form. You must print this requisition form out and bring it to your blood draw. See an example of the email HERE.

How will I know where to go for my blood draw?

DHA Laboratory will send you a list of the closest Labcorp locations to your address. This list will come in the email that includes your test requisition form. You can also visit https://www.labcorp.com/labs-and-appointments to find the nearest Labcorp locations to you.

Please complete your draw within 6 months of receiving your requisition form. LabCorp will not draw requisition forms older than 6 months old.

Do I need an appointment?

To schedule your blood draw, visit https://www.labcorp.com/labs-and-appointments.

Do I need to pay for my blood draw?

No, Labcorp will charge DHA Laboratory directly for your blood draw. You SHOULD NOT pay for your blood draw.

Can I get my test drawn somewhere other than LabCorp?

No, you should only get your blood drawn at Labcorp, UNLESS DHA Laboratory has given you another option in the information email.

If I am also collecting a DHA Laboratory urine test, do I bring that with me to my blood draw?

DO NOT bring your urine sample with you to your blood draw. You should send your urine sample back to the lab using the shipping materials included in the urine test kit.
