Requisition forms are like IDs for your samples and let the lab know which tests to perform. Some labs may call it a "Lab Order" or "Lab Form." Don't worry, we've already filled out most of the information when you ordered your test kit.

You can find SAMPLE requisition forms for each laboratory we partner with on our list here. Our sample form will show you exactly what sections to fill out (like the date of collection) and which sections are already taken care of.

Sending tests without this form runs the risk of your samples being discarded and a full recollection being required.
