If you are taking any of the following medications, consult your healthcare provider regarding timing.

Non-essential nutritional supplements
You must fast overnight before your blood draw.
You must freeze the included freezer brick for a minimum of 8 hours. Bring it with you to the blood draw.
If you have the Methylation Genomics add-on, complete it before the blood draw.
Collection Instructions
Freeze the included freezer brick for a minimum of 8 hours prior to your blood draw.
Complete the test registration (instructions HERE).
Please ensure that the name on the registration matches the name on your Rupa order EXACTLY. If it does not match, you may need to recollect your samples.
If you have the Methylation Genomics add-on, complete the test prior to your blood draw.

Bring the following items with you to your blood draw appointment.
Test kit
Frozen freezer brick
Completed activation card
Methylation Genomics swabs (if you paid for the add-on)
The phlebotomist will collect, prepare, and ship your sample for you.

NOTE: If you are in Alaska or Hawaii, Genova recommends shipping with dry ice to ensure your samples arrive frozen, as overnight shipping is not guaranteed due to the distance. If your samples do not arrive frozen, you may need to recollect. Please note on your activation card if you use dry ice.
Requisition & Payment
Your kit has been prepaid. You may need to pay a separate fee for phlebotomy.

Since you pre-paid for your tests, you do not need to fill in the payment information on this form.
Shipping
Make a note of the tracking number on the return mailer so that you can track your samples back to the lab.
The phlebotomist will ship your sample for you using the materials provided in the test kit.

Results
Results should be released to your provider 7-21 business days after samples are received at the lab. We cannot guarantee processing time.
Your provider will notify you when your results are in and schedule a follow-up appointment for you to review them together.
