Bring the following items with you to your blood draw appointment.

The phlebotomist will collect, prepare, and ship your sample for you.
Requisition & Payment
Your kit has been prepaid. You may need to pay a separate fee for phlebotomy.
You must complete your requisition form and send it back with your samples.
Since you pre-paid for your tests, you do not need to fill in the payment information on any of this form.
Shipping
Make a note of the tracking number on the return mailer so that you can track your sample back to the lab.
Your sample will be shipped by the phlebotomist back to Alletess with the included packing materials and return mailer.
Results
Results should be released to your provider 5-7 business days after samples are received at the lab. Rupa Health cannot guarantee processing time.
