This test measures the cortisol awakening response. It can be added to any of the following tests: Adrenocortex Stress Profile, Menopause Plus™, Rhythm Plus™, Male Hormones Plus™, and One Day Hormone Check™. This test MUST be collected on the same day as the test to which it was added.

Your samples MUST be collected on the same day as the test to which it was added.
Ship your samples as soon as possible.
Rupa Health recommends shipping Monday - Thursday only to avoid shipment delays.
Preparation & Timing
You will collect these samples on the same day as you collect the 4-5 samples for the parent test.
Specific preparation and timing information will be determined by the parent test. Please see specific preparation and timing information below:
The night before collection, set out the supplies for your first morning collection.
Collection tube
Glass of water
Low-level light
DO NOT turn on a bright light when you first wake up.
Avoid the following activities for 1 hour prior to collection:
Eating
Drinking anything except water
Brushing your teeth
Flossing your teeth
Using mouthwash
Chewing gum
Using tobacco products

Collection Instructions
The night before collection, place the following items by your bed.
Collection tube
Glass of water
Low-level light
Immediately upon waking, collect your first sample.
Fill the collection tube with saliva until you reach the 3-ml mark.
You should finish the collection within 5 minutes of starting.
Your sample should not contain bubbles or mucus.
Replace the cap tightly.
Write the following information on the appropriate label (ie: label 1 for collection 1). Attach the label to the collection tube.
Your first and last name - Please ensure that the name on the label matches the name on your Rupa order EXACTLY. If it does not match, you may need to recollect your samples.
Time you started collection
Date of collection
Place the tube into the biohazard bag.
Freeze the tube (in the bag) immediately.
Samples should be frozen a minimum of 2 hours prior to shipping.
Keep samples frozen until ready to ship.
Repeat steps 3-7 for each collection (schedule HERE).
You will have additional collections for the parent test. Collect these on the same day. See collection instructions for those tests here.
Complete the test registration (instructions HERE).
Please ensure that the name on the registration matches the name on your Rupa order EXACTLY. If it does not match, you may need to recollect your samples.
Requisition & Payment
Your kit has been prepaid. You DO NOT need to pay anything extra.

Since you pre-paid for your tests, you do not need to fill in the payment information on this form.
Shipping
Ship your samples as soon as possible.
Rupa Health recommends shipping Monday - Thursday only to avoid shipment delays.
Ship your samples via FedEx.
You can take your samples to the nearest FedEx location.
DO NOT place your samples in a drop box.
You can also schedule a FedEx pickup by calling 1-800-463-3339 or visiting their website www.fedex.com.
To prepare your samples for shipping,
Place the frozen sample tubes into the biohazard bag.
Place the biohazard bag into the test kit box.
Place the test kit box into the provided return mailer.
Place the completed activation card into the provided return mailer.
Make a note of the tracking number on the label so that you can track your samples back to the lab.
Results
Results should be released to your provider 7-21 business days after samples are received at the lab. We cannot guarantee processing time.
Your provider will notify you when your results are in and schedule a follow-up appointment for you to review them together.
