The Lyme Multiplex PCR test detects B. burgdorferi infection through a urine sample.


Key Details

  • ❌ DO NOT collect while menstruating.

Collection Instructions

  1. Complete the test requisition form.

  2. Wash your hands with soap and warm water.

  3. Dry your hands with a clean paper towel.

  4. Take the urine collection cup out of the test kit.

    1. DO NOT touch the inside of the cup.

  5. Open the towelette and wipe yourself.

    1. If you are female, separate your labia with your fingers and clean inside with the towelette, moving from front to back.

    2. If you are male, wipe the end of your penis with the towelette.

  6. Allow a small amount of urine to enter the toilet to eliminate any contaminants.

  7. Collect your mid-stream urination in the collection cup until it is filled.

  8. Pass any remaining urine into the toilet.

  9. Open one of the gray-top tubes.

  10. Use the provided pipette to transfer urine from the collection cup to the gray-top tube.

  11. Fill the tube past the minimum fill line (indicated on the label).

  12. Place the pipette back into the urine collection cup.

  13. Cap the tube tightly.

  14. Gently invert the tube 8-10 times to mix the preservative with the specimen.

  15. Write the following information on one of the white tube labels.

    1. Full name

    2. Date of birth

    3. Date of collection

  16. Place the label on the gray-top tube.

  17. Discard any remaining urine.

  18. Place the gray-top tube into the white tube holder.

  19. Place the tube holder into the biohazard ziplock bag and seal.

  20. Place the ziplock bag into the bubble pouch.

  21. Place the bubble pouch into the specimen kit box.

  22. Place the specimen kit box into the refrigerator.


Requisition & Payment

  • Your kit has been prepaid. You DO NOT need to pay anything extra.

YOU MUST PRINT YOUR TEST REQUISITION FORM FROM YOUR RUPA PATIENT PORTAL.

  • This form was also emailed to you from the Rupa Health team.

DO NOT use the blank requisition form that came in your test kit, as it does not have all of the required information. If you use the form from your kit, you may be required to recollect your sample.

  • You must complete your requisition form and send it back with your sample.

    • Since you pre-paid for your tests, you do not need to fill in the payment information on this form.


Shipping

  • To prepare your sample for shipping,

    • Place the gray-top tube into the white tube holder.

    • Place the tube holder into the biohazard ziplock bag and seal.

    • Place the biohazard bag into the bubble pouch.

    • Place the bubble pouch into the specimen kit box.

    • Place the completed test requisition form into the specimen kit box.

    • Place the specimen kit box into the FedEx Pak.

    • Seal the FedEx Pak.

  • Make a note of the tracking number so that you can track your sample back to the lab.

    • The receipt enclosed with your FedEx pack will contain the tracking number.

  • Ship your sample via FedEx as soon as possible after collection.

    • Take the package to your nearest FedEx location to ship. Visit www.fedex.com to find your nearest location.

    • DO NOT place your sample in a FedEx drop box.


Results

  • Results should be released to your provider 10 business days after samples are received at the lab. Rupa Health cannot guarantee processing time.
