Quest Diagnostics's blood panels can include any combination of biomarkers chosen for you by your practitioner.

Key Details

  • 🏥 Sample collection must be performed by a phlebotomist, Monday - Friday only.

  • 💰 You WILL NOT need to pay a separate fee for phlebotomy.

  • 📦 You WILL NOT receive a test kit for your blood draw.

  • 📄 Your requisition form will be emailed to you. You must bring your requisition form (printed or on your phone) to your blood draw.

  • 🩸 Your blood draw will be performed at Quest Diagnostics. Rupa Health recommends making an appointment for your blood draw.

  • ❗️Some tests require special preparation.

Collection Instructions

  1. Read this article to prepare for your blood draw.

  2. Your blood draw will be performed at Quest Diagnostics. Rupa Health recommends making an appointment for your blood draw.

  3. Bring your test requisition form with you to your blood draw appointment.

    1. Rupa Health will email you this form.

    2. You may print the form out or show it on your mobile device.

  4. The phlebotomist will collect, prepare, and ship your sample for you.

Requisition & Payment

  • Your kit has been prepaid. You WILL NOT need to pay a separate fee for phlebotomy.

YOUR REQUISITION FORM CAN BE FOUND IN YOUR RUPA PATIENT PORTAL. RUPA HEALTH WILL ALSO EMAIL IT TO YOU.

Please bring this form with you to your blood draw.

Shipping

  • The phlebotomist will ship your sample for you.

Results

  • Results should be released to your provider 1-10 business days after samples are received at the lab. Rupa Health cannot guarantee processing time.
