This panel provides an assessment of four hormones that regulate fertility health.
Key Details
🧪 You will collect a saliva sample and a blood spot sample.
🩸 You will need to time your collection appropriately with your menstrual cycle.
💊 If you are taking hormones, you may need to time your test appropriately around your doses.
❌ You must avoid certain foods and activities on the day of saliva sample collection.
📦 Ship your samples as soon as possible after collection.
Preparation & Timing
You will collect one saliva sample and a blood spot sample.
You must collect your samples within 30 minutes of waking for the day.
If you are a non-menstruating female, you can collect your samples on any day.
If you are a menstruating female,
Regular cycles: Collect samples between days 19-21 of your cycle (where day 1 is the first day of your period).
Irregular cycles: Collect samples any day you are not bleeding.

DO NOT collect your samples if you are sick.
You can continue to take most of your supplements and vitamins. Please note any supplements you take on collection day on the requisition form.
If you are taking any of the following hormones, collect your sample halfway between doses.
Hormone injections
Hormone pellets
Transdermal patch hormones
5 Days Before Sample Collection
Stop using cortisol-containing products.
Talk to your healthcare provider about timing if you are taking cortisol for adrenal support.
3 Days Before Sample Collection
DO NOT use any over-the-counter anti-aging creams.
Stop using 7-keto-DHEA.
Consult your healthcare provider before discontinuing any prescription medications.
24 Hours Before Sample Collection
Stop using topical and sublingual hormones.
Stop using oral hormones (except progesterone).
Consult your healthcare provider before discontinuing any prescription medications.
10 Hours Before Sample Collection
Stop using oral progesterone.
Consult your healthcare provider before discontinuing any prescription medications.
Day of Sample Collection
DO NOT drink alcohol.
DO NOT use any lip products (lipstick, chapstick, etc.).
DO NOT take any hormones.
Consult your healthcare provider before discontinuing any prescription medications.
60 Minutes Before Sample Collection
DO NOT eat a big meal.
30 Minutes Before Sample Collection
DO NOT eat.
DO NOT drink anything but water.
DO NOT brush your teeth.
DO NOT exercise.
DO NOT smoke or use tobacco products.
Collection Instructions
You will collect TWO different samples.
Saliva Collection
Rinse your mouth with water 5 minutes before sample collection.
Wash your hands and dry them with a clean towel.
Open the kit and get only one collection tube.
Close the kit immediately.
Unscrew the cap on the tube.
Pool saliva in your mouth.
To help produce saliva, try
Yawning
Thinking of your favorite food
Smelling lemons or lemon juice
Tilt your head forward.
Push saliva down into the tube using your tongue.
Fill the tube at least 3/4 full (excluding bubbles).
Screw the cap tightly on the tube.
Write the following information on the tube.

Place the sample tube inside the ziplock bag.
Close the ziplock bag tightly.
Place the ziplock bag into the freezer.
Each sample tube will be added to this bag.
Samples will stay in the freezer until shipped.
Write the following information in section 7 of the test requisition form.

Blood Spot Collection
Open the provided collection card and lay it on a flat, clean surface.
DO NOT touch the collection circles.
Write the following information on the card.
Full name
Date of collection
Time of collection
Wash your hands with warm water.
Dry your hands with a clean towel.
Try one of the following activities to increase circulation and improve blood flow.
Rubbing your hands together
Swinging your arms around several times
Shaking your arms
Doing a few jumping jacks
Choose the finger you will prick.
It is recommended that you use your middle or ring finger on your non-dominant hand.
Use the alcohol pad to clean the chosen finger.
Allow your finger to dry for 20-30 seconds.
Open one of the lancets by twisting off the cap.
Press the small end of the lancet firmly against the SIDE of your finger until you hear a CLICK.
DO NOT prick your finger pad.
Wipe away the first drop of blood with the sterile gauze pad.
Position your finger above one of the circles on the collection card.
Gently massage the entire length of your finger to form a large, hanging blood drop.
DO NOT squeeze your finger or touch your fingertip.
Touch the blood drop to the center of the circle on the card.
DO NOT touch the card with your finger.
Fill all of the circles on the card.
Only ONE blood drop per circle.
If you cannot get enough blood with the first finger, repeat steps 5-15 with another finger and the second lancet to complete the card.
When all the circles have been filled, wipe away the blood from your finger using the gauze pad and apply the bandage.
Leave the collection card open to dry for at least 30 minutes.
Complete the test requisition form.
Once the card is dry, close the top flap.
Place the card into the biohazard pouch.
Place the biohazard pouch into the refrigerator until ready to ship.
Requisition & Payment
Your kit has been prepaid. You DO NOT need to pay anything extra.
You must complete your requisition form and send it back with your samples.
Since you pre-paid for your tests, you do not need to fill in the payment information on this form.
Shipping
Ship your sample as soon as possible after collection.
Keep your sample in the refrigerator until ready to ship.
To prepare your sample for shipping,
Close the top flap on the blood spot card once it is dry.
Place the collection card into the biohazard pouch.
Seal the biohazard pouch.
Place the biohazard pouch into the test kit box.
Remove the ziplock bag containing the saliva samples from the freezer.
Place the ziplock bag into the test kit box.
Place the completed test requisition form into the test kit box.
Place the test kit box into the provided return mailing envelope.
Seal the envelope.
Attach the return label to the envelope.
Make a note of the return tracking number on the label so that you can track your sample back to the lab.
Results
Results should be released to your provider 3-5 business days after your sample is received at the lab. Rupa Health cannot guarantee processing time.
