There are two forms you must fill out:

  1. Patient Health Questionnaire

  2. Consent Form

Patient Health Questionnaire

You should have received an email from Microbiome Labs entitled "BiomeFX Lab Ordered." Use the link in that email to fill out the questionnaire.

You will need your sample ID number, which can be found on the inside of your test kit box.

Consent Form

This consent form will be emailed to you from Microbiome. It will be titled "Consent Required for Your BiomeFx Lab Test."

Please complete this form so that your sample can be processed at the laboratory. Failure to complete this consent may result in delayed results or a recollection.
